When you make an online purchase using www.IndplsArtCenter.org, the Art Center will ask for the following information ("personally identifiable information", or "PII") from you:
Credit/Debit Card number, cardholder name and expiration date
Your telephone/fax number

Specific information required includes your full name, address, telephone number and e-mail address.
At the Art Center's request, you may also choose to voluntarily provide additional PII to the Art Center, such as your birth date, gender, marital status, etc. The Art Center will use the PII provided by you to create an account profile for your Art Center frequent shopper account. The Art Center may also gather PII through your voluntary subscription to the Art Center's e-mail specials, and your voluntary participation in surveys, polls, promotions, contests, and sweepstakes sponsored by the Art Center.
The Art Center collects PII in order to: (1) tailor your experience at IndplsArtCenter.org to your interests; (2) help the Art Center continually refine IndplsArtCenter.org and improve the services offered by the Art Center; (3) provide you with information about promotions and offers through e-mail and at IndplsArtCenter.org that may be of interest to you; and (4) allow the Art Center to contact you for customer service, if necessary.
